The HAP Foundation, in partnership with Education in Palliative and End of Life Care (EPEC), is offering a 12-month clinical training program focused on adult palliative care in home- and community-based settings, specifically designed for rural Illinois providers.
This clinically focused program includes monthly virtual training sessions via Zoom, along with ongoing networking and collaborative learning opportunities throughout the year. Sessions will be led by Dr. Joshua Hauser, Dr. Gordon Wood, Sara Dado, LCSW, and other regionally and nationally recognized experts in the field of home- and community-based palliative care.
The cohort year will begin and conclude with full-day, in-person educational events. Between these in-person sessions, participants will meet virtually for case sharing, structured educational modules, and peer networking. Curriculum topics will include advance care planning, having difficult conversations, whole-patient assessment, pain and symptom management, goals of care, effective use of the interdisciplinary team, and cultural competence in home-based care. Virtual sessions will provide opportunities for participants to present cases and receive peer support and clinical guidance from a team of expert faculty.
We are encouraging nurse practitioners, physician assistants, registered nurses, social workers, community health workers, certified nursing assistants and emergency service personnel to take part in this opportunity.
The program will launch with a full-day, in-person conference on April 17, 2026, in central Illinois. Additional details will be shared soon.

The HAP Foundation has identified the need for additional palliative care clinical education focused on health care professionals of the interdisciplinary team who are providing care in home settings across a variety of communities in Illinois. While many of these professionals have clinical expertise, they may lack specific knowledge in understanding the nuances needed to effectively manage serious and chronically ill patients in the home setting.
