Creating Communities of Care for Seriously Ill Children

Join us and the Children’s Hospital of Illinois (CHOI) Team to learn more about the new Illinois law that creates reimbursement for children receiving home and community based palliative care and begin building your team’s confidence in caring for children. Parents will also share their stories and the powerful experience of partnering with palliative care and hospice teams in their journey caring for their seriously ill child. Additionally, we are eager to introduce you to The HAP Foundation team and new initiatives occurring with adult serious illness care. This event is a great opportunity to network with others who are committed to improving the lives of seriously ill adults and children and their families.
